II. Diagnosis and Treatment of Key Diseases
1. Knee Osteoarthritis (KOA)
- Western Medicine Definition: A degenerative change of the knee joint, manifested as pain, swelling, limited movement, and osteophyte formation.
- Traditional Chinese Medicine (TCM) Category: Belongs to the category of "Bi Syndrome" or "Bone Bi Syndrome" in TCM. The causes include liver and kidney deficiency, long-term overwork, and exogenous wind-cold-dampness.
- Treatment Principles: Relieve pain, alleviate symptoms, prevent and delay disease progression, protect joint function, etc.
- Treatment Methods: Comprehensive Western medicine therapy, comprehensive TCM therapy, and integrated traditional Chinese and Western medicine therapy, including health education, internal and external use of drugs, physical therapy, and surgical operations.
2. Avascular Necrosis of the Femoral Head
- Western Medicine Characteristics: A common chronic orthopedic disease. Interruption of blood supply to the femoral head leads to microstructural changes, and eventually collapse and deformation of the femoral head.
- TCM Pathogenesis: Conforms to the theory of "Deficient Qi Stagnation", which refers to deficiency of primordial Qi (deficient Qi) and stagnation of Qi, blood stasis, dampness accumulation and phlegm coagulation (stagnation).
- Treatment Principles: Tonify deficiency and cultivate primordial Qi, relieve stagnation and dredge obstruction (Cultivate Primordial Qi and Dredge Obstruction). The priority is to tonify deficiency, cultivate primordial Qi and consolidate the root, and the treatment involves promoting Qi circulation, activating blood circulation and resolving phlegm.
- Treatment Methods: Self-formulated Decoction for Cultivating Primordial Qi and Consolidating the Root (for soothing the liver and regulating the spleen to adjust Qi movement, activating blood circulation and dredging collaterals, etc.), combined with Western medicine methods such as health education, internal and external use of drugs, and surgical operations.
3. Lumbar Disc Herniation
- Western Medicine Mechanism: On the basis of intervertebral disc degeneration, the annulus fibrosus ruptures and the nucleus pulposus protrudes, compressing the nerve roots, resulting in lumbocrural pain and neurological dysfunction.
- TCM Category: Scattered in the categories of "Lumbocrural Pain" and "Bi Syndrome" in TCM. The key pathogenesis is deficiency of kidney Qi and malnutrition of muscles and bones; the inducing factors are falls, sprains or invasion of cold-dampness; the core is obstruction of meridians and unsmooth circulation of Qi and blood.
- Treatment Methods: Combination of Western medicine arthroscopic minimally invasive diagnosis and treatment technology and TCM syndrome differentiation and treatment.
4. Shoulder Bi Syndrome (including Periarthritis of Shoulder, Rotator Cuff Injury, etc.)
- Western Medicine Characteristics: Diseases characterized by pain around the shoulder joint, accompanied by limited functional activities.
- TCM Pathogenesis: The disease location is in the shoulder muscles, related to the Three Yang Meridians of Hand and the Taiyin Meridian of Hand. It is caused by unsmooth meridians, malnutrition of muscles, invasion of wind-cold-dampness pathogens, muscle and tendon injury due to overwork, and Qi and blood deficiency in the elderly.
- TCM Treatment:
- Acupuncture: Promotes the circulation of meridian Qi, activates blood circulation and removes blood stasis.
- Internal Use of TCM: Taoren Siwu Decoction, Bazhen Decoction, etc.
- TCM Massage: Regulates tendons to relieve pain, activates blood circulation and removes blood stasis.
- Department Features: Integrates Western medicine minimally invasive technology with TCM tendon regulation, acupuncture and massage, combined with internal use of TCM.
